Hypertension is one of the most common non-communicable diseases leading to complications. Higher health literacy is effective in controlling blood pressure. This randomized controlled trial using single-blind method aimed to examine the effectiveness of Health Education and Health Empowerment Program to improve health literacy among Thai adults. Participants were randomly assigned to a control group for usual care (n = 20) and intervention group for HEaHEP during 4 weeks (8 sessions) (n = 20) at a subdistrict health promotion hospital. The Health Literacy for Hypertension questionnaire [HLHQ] was used to measure primary outcome variable across three times points at baseline (week 0), post- intervention (week 4) immediately after completed the intervention and follow-up (week 8) between two groups. Two-way mixed ANOVA (one-within subject and one-between groups) were used to analyze the data. The results indicated that the intervention group had better HLH score than control group at three time measured were statistic significant difference at post-intervention and follow-up period (F1,38 = 51.03, p < .001 and F1,38 = 53.39, p < .001). The mean scores of HLH between intervention and control group were different over time (F1,19 = 112.24, p < .001 and F 1,19 = 15.99, p < .001). the mean score of health literacy in week 4 and follow-up period (week 8) were higher than pre-intervention period (week 0) (Mdiff = -39.95, p < .001, Mdiff = - 45.00, p < .001). These findings indicate that the HEaHEP effectively improved health literacy in adults with hypertension over time. The nurse practitioners can apply this program to improve health literacy in adult with hypertension in primary health care.en
